Introduction
Low testosterone symptoms can affect sexual function, energy, mood, muscle mass, and overall well-being. However, these symptoms are not specific to testosterone deficiency. Fatigue, reduced libido, erectile difficulties, changes in body composition, and mood changes can also occur with sleep disorders, obesity, thyroid disease, medication use, chronic illness, stress, and other health conditions.
For that reason, recognizing possible symptoms is only the first step. Testosterone deficiency should be diagnosed through an appropriate medical evaluation that considers symptoms, medical history, physical findings, and consistently low testosterone levels confirmed with laboratory testing.
In this guide, we’ll review five important low testosterone symptoms, other conditions that can cause similar problems, how testosterone deficiency is properly tested, and when treatment may be appropriate.

1. Persistent Fatigue Despite Rest
Persistent fatigue and decreased energy are among the possible low testosterone symptoms, although they are not specific to testosterone deficiency. Some men may notice reduced motivation, lower physical endurance, increased sleepiness, or difficulty maintaining their usual level of activity.
However, fatigue has many potential causes. Poor sleep, obstructive sleep apnea, anemia, thyroid disorders, depression, chronic illness, certain medications, obesity, and other health conditions may produce similar symptoms.
For this reason, persistent fatigue should not automatically be attributed to testosterone. When reduced energy occurs together with other suggestive symptoms—particularly changes in sexual function—an appropriate medical evaluation can help determine whether testosterone testing is warranted.
This aligns with the Endocrine Society guideline, which classifies decreased energy, motivation and increased sleepiness among the nonspecific findings associated with testosterone deficiency.
2. Low Libido and Reduced Performance
Changes in sexual function are among the more clinically relevant symptoms associated with testosterone deficiency. Men may notice a persistent decrease in sexual desire, fewer spontaneous or morning erections, or changes in erectile function.
Possible changes may include:
- Reduced interest in sexual activity
- Fewer spontaneous or morning erections
- Difficulty achieving or maintaining erections
- Reduced sexual satisfaction
However, erectile dysfunction does not automatically mean testosterone is low. ED can also be associated with cardiovascular disease, diabetes, vascular problems, medications, psychological factors, and other conditions.
When reduced libido or erectile difficulties occur—particularly when accompanied by other low testosterone symptoms—testing may be appropriate as part of a broader medical evaluation.
The Endocrine Society specifically identifies reduced libido, decreased spontaneous erections, and erectile dysfunction among symptoms suggestive of testosterone deficiency.
3. Loss of Muscle Mass and Increased Fat
Testosterone contributes to the maintenance of muscle mass, strength, and body composition. Men with testosterone deficiency may notice gradual changes in their physique, particularly when low testosterone persists over time.
Possible changes may include:
- Reduced muscle mass or strength
- Difficulty maintaining previous levels of physical performance
- Increased body fat
- Changes in overall body composition
These changes are not unique to low testosterone. Aging, reduced physical activity, inadequate nutrition, obesity, chronic illness, and other metabolic or hormonal conditions can produce similar effects.
When changes in muscle mass or body composition occur together with sexual symptoms, persistent fatigue, or other low testosterone symptoms, a medical evaluation can help determine whether testosterone deficiency or another condition may be contributing.
The AUA also recognizes reduced energy, endurance, and physical performance among the symptoms clinicians may encounter when evaluating testosterone deficiency.
4. Mood Swings and Mental Fog
Testosterone deficiency may be associated with changes in mood, motivation, and cognitive function. Some men report feeling less motivated, more irritable, or having greater difficulty concentrating than usual.
Possible symptoms may include:
- Reduced motivation or initiative
- Irritability or changes in mood
- Difficulty concentrating or problems with memory
- Reduced sense of well-being
- Depressed mood in some men
These symptoms have many possible causes, including chronic stress, depression, anxiety, poor sleep, medications, and other medical conditions. Therefore, mood or concentration problems alone should not be considered evidence of testosterone deficiency.
When these changes occur together with more suggestive symptoms—such as reduced libido or fewer spontaneous erections—and persist over time, a broader medical evaluation may be appropriate.
The Endocrine Society lists decreased motivation, depressed mood, poor concentration, memory difficulties, and sleep disturbance among the nonspecific symptoms associated with testosterone deficiency.
5. Poor Sleep and Recovery
Men with testosterone deficiency may experience changes in sleep, including disturbed sleep or increased daytime sleepiness. However, the relationship between testosterone and sleep is complex, and sleep problems should not automatically be attributed to low testosterone.
Possible concerns may include:
- Difficulty maintaining restful sleep
- Increased daytime sleepiness
- Feeling unrefreshed despite adequate time in bed
- Reduced energy and concentration associated with poor sleep
Importantly, sleep disorders themselves—particularly obstructive sleep apnea—can affect overall health and may also influence testosterone levels. Obesity, medications, stress, alcohol use, and other medical conditions can further contribute to both sleep problems and hormonal changes.
For this reason, persistent sleep problems deserve appropriate evaluation rather than assuming testosterone is the underlying cause. When sleep disturbance occurs together with sexual symptoms and other low testosterone symptoms, testosterone testing may be one part of a broader clinical assessment.
How Is Low Testosterone Diagnosed?
Having one or more low testosterone symptoms does not necessarily mean that testosterone levels are low. Because many of these symptoms overlap with other health conditions, diagnosis requires both a clinical evaluation and appropriate laboratory testing.
A typical evaluation may include:
- Medical history and symptom review — including sexual function, energy, medications, sleep, fertility goals, and other health conditions.
- Early-morning total testosterone — testosterone should generally be measured in the morning, when levels are typically highest.
- Repeat testosterone testing — an initially low result should generally be confirmed with a second early-morning measurement on a separate day.
- Free testosterone when appropriate — this may be useful when total testosterone is borderline or when conditions affecting sex hormone-binding globulin (SHBG) may make total testosterone more difficult to interpret.
- LH and FSH — when testosterone deficiency is confirmed, these hormones can help determine whether the problem is primarily testicular or related to the pituitary or hypothalamus.
- Additional laboratory evaluation — depending on the individual, further testing may be appropriate to investigate other potential causes of the symptoms or low testosterone.
Current Endocrine Society guidance recommends diagnosing hypogonadism only when a man has compatible symptoms or signs and unequivocally and consistently low testosterone levels. It also recommends confirming the diagnosis with a repeat morning fasting total testosterone measurement.
What Else Can Cause Low Testosterone Symptoms?
Many symptoms associated with low testosterone can also occur with other medical conditions or lifestyle factors. Identifying these potential contributors is important because treating the underlying cause may improve symptoms without testosterone therapy.
Common contributors may include:
- Obesity and metabolic dysfunction — excess body weight can be associated with lower testosterone levels and may contribute to fatigue and changes in body composition.
- Poor sleep or obstructive sleep apnea — sleep disorders can cause fatigue, reduced concentration, sexual difficulties, and hormonal changes.
- Thyroid disorders — abnormal thyroid function can affect energy, mood, weight, and sexual function.
- Depression, anxiety, and chronic stress — these can affect energy, motivation, sleep, concentration, and libido.
- Certain medications — opioids and glucocorticoids, among others, can interfere with normal testosterone production.
- Chronic medical conditions — diabetes, systemic illness, and other health problems may produce symptoms that overlap with testosterone deficiency.
- Excessive alcohol use — heavy alcohol consumption can negatively affect reproductive and overall health.
When Is Testosterone Treatment Appropriate?
Testosterone therapy should not be based on symptoms alone or on a single testosterone result. Treatment may be considered when a man has compatible symptoms or signs, consistently low testosterone levels confirmed by appropriate testing, and a clinical evaluation supporting a diagnosis of testosterone deficiency.
Before starting treatment, potential underlying causes should be evaluated. In some men, addressing reversible contributors—such as obesity, certain medications, sleep problems, or other medical conditions—may improve testosterone levels or symptoms without testosterone replacement therapy. For obesity-associated hypogonadism without another identified cause, the Endocrine Society’s 2026 statement describes weight loss as typically first-line therapy.
When testosterone replacement therapy (TRT) is clinically appropriate, treatment should be individualized based on symptoms, laboratory findings, medical history, potential benefits and risks, and the patient’s goals.
Fertility is particularly important. Testosterone therapy can suppress sperm production and should generally not be started in men planning fertility in the near term. Other contraindications and precautions also need to be assessed before treatment begins.
Men receiving TRT require ongoing follow-up to evaluate symptom response, testosterone levels, hematocrit, adverse effects, and other appropriate safety parameters.
When Should You See a Doctor for Low Testosterone Symptoms?
Consider discussing your symptoms with a qualified healthcare professional if they are persistent, worsening, or affecting your sexual health, energy, mood, physical performance, or quality of life.
Evaluation may be particularly appropriate if you experience:
- A persistent decline in libido
- Fewer spontaneous or morning erections
- Ongoing erectile difficulties
- Unexplained loss of muscle mass or strength
- Persistent fatigue accompanied by other symptoms
- Changes in mood, motivation, or concentration
- Fertility concerns
- Several low testosterone symptoms occur together
A medical evaluation can help determine whether testosterone deficiency, another hormonal condition, medication use, sleep problems, metabolic disease, or another health issue may be contributing.
Importantly, testosterone treatment should not be started simply because symptoms are present. Current guidance emphasizes establishing an accurate diagnosis with symptoms plus consistently low testosterone measurements and evaluating potentially reversible causes before deciding whether TRT is appropriate.

Low testosterone symptoms can affect sexual function, energy, mood, sleep, muscle mass, and overall well-being. However, experiencing these symptoms does not automatically mean that testosterone is the cause.
The most appropriate approach is to evaluate symptoms together with medical history, physical findings, medications, lifestyle factors, and properly performed laboratory testing. Current clinical guidance recommends confirming testosterone deficiency with consistently low testosterone measurements rather than relying on symptoms or a single laboratory result.
When testosterone deficiency is confirmed, treatment should be individualized based on the underlying cause, symptoms, fertility goals, potential benefits and risks, and the patient’s overall health.
